Boarding Contract

    Todays Date

    I (hereinafter referred to as owner”) (hereinafter referred to as pet) in exchange for the goods
    and/or services provided by Bones: Bed, Bath, Biscuits & More, LLC (hereinafter referred to as
    “Bones”), hereby agree to the following terms and conditions and certify the truthfulness of all
    information I have provided regarding my pet(s).

    1. REPRESENTATIONS ABOUT MY PETS:
    **I hereby certify my pet(s) are current on ALL VACCINATIONS. I hereby certify that my pet(s) is/are
    in good health and has/have not been exposed to rabies, distemper, or any other communicable condition
    within a thirty (30) day period prior to boarding.
    *I understand that pets known to be aggressive towards other dogs or people are strictly prohibited from
    Bones facilities. I hereby certify that my pet(s) has/have not shown aggressive or threatening behavior
    towards any other pet or any person.
    **I understand that, in admitting my pet(s), Bones has relied on my representation that my pet(s) is/are in
    good health, does not currently have and has not had within the last (30) days any communicable diseases,
    and has/have not harmed or shown aggressive or threatening behavior towards any person or any other
    pet.

    2. PET HEALTH
    **I understand that Bones holds other pets, including pets from other boarding clients. Although Bones
    does everything possible to ensure your pet stays healthy, there is always the possibility of contagious
    illnesses being spread. In the event of an emergency, Bones will NOTIFY Owner of emergency as soon
    as reasonably possible (with deference given to the urgency required for treatment of pet) at the contact
    information that I have provided. If I am unreachable and my pet(s) becomes ill, Bones, in its SOLE
    DISCRETION, may engage the services of a veterinarian, or administer medicine or give other requisite
    attention to the animal.
    I hereby agree to REIMBURSE Bones for veterinary costs, whether specifically authorized or not, in an
    amount that will be unlimited unless otherwise limited by Owner to an amount no greater than (Select
    One):

    ****For Senior and Chronically Sick Pets: I understand that there are inherent risks in boarding a senior
    or chronically sick pet. Underlying health problems can be exacerbated by stress, and illnesses or
    infections, which would be minor in a healthy pet, can become serious and even life-threatening in an
    elderly or frail pet. If my pet falls ill at Bones Veterinary, care and intervention (INCLUDING final
    arrangements, if necessary) are as follows:

    ****My pet(s) is/are allergic to the following medications, food, and other allergens. Give name(s) of
    each pet!

    3. SERVICES
    **PLAY GROUPS. I understand that allowing my pet to participate in a Play-Group may
    involve certain risks including, but not limited to, injury or death to my pet(s), other client’s pets,
    and to the employees of Bones. I realize
    that when pets are around people and other dogs, accidents or injuries may result, and that people, pets and
    property could be injured or damaged.
    **PET TAXI IN CASE OF EMERGENCY!! I understand there are inherent dangers in
    transporting my pet. I understand the driver, vehicle, owner, Bones, its directors, officers,
    shareholders, employees, assistants, members and agents are NOT responsible for any
    injury/damages which may be incurred in transporting my pet(s)and in consideration for providing
    transportation, I agree to hold Bones and all if its owners, employees, assistants, members, agents,
    directors, officers, shareholders, as well as the owners and drivers of the vehicle transporting my
    pet(s), harmless from claims for injury or damages occurring during said trip/Pet Taxi.

    4.FEES, TERMS AND CONDITIONS:
    **I agree to pay the rate for boarding in effect on the date my pet is checked in, and all costs and charges
    for special services requested, or deemed necessary by Bones. All Charges incurred shall be due and
    payable upon pick-up of pet. RATE SUBJECT TO CHANGE WITHOUT NOTICE!
    **I understand that Bones reserves the right to refuse to board any pet for any reason at any time.
    **I understand that no one, aside from Bones staff and volunteers, will be allowed access to my pets
    unless their name has been provided in this agreement.
    **This Contract contains the entire agreement between the parties. All terms and conditions of this
    Contract shall be binding on the heirs, administrators, personal representatives, and assigns of the Owner
    and the Kennel.
    **Any controversy or claim arising out of or relating to this contract, or the breach thereof, or as the
    result of any claim or controversy involving the alleged negligence by any party to this Contract, shall
    be settled in accordance with the rules of the American Arbitration Association, and judgment upon the
    award rendered by an arbitrator may be entered in any court having jurisdiction thereof. The arbitrator
    shall, as part of his award, determine an award to the prevailing party of the costs of such arbitration and
    reasonable attorney’s fees of the prevailing party.

    5. RELEASE & WAIVER INDEMNIFICATION:
    **I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are
    boarded at Bones. Such harm includes but is not limited to damage or injury to the pets of other clients,
    the employees of Bones, and to any part of Bones’ facility.
    **I hereby release and agree to save and hold harmless, Bones, its directors, officers, shareholders,
    employees, assistants, members, and agents from any and all liability, claims, suits, actions, loss, injury,
    or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury, or damage which
    I or my dog(s) may sustain or which may be caused in any way by my dog(s). I specifically, without
    limitation, agree to fully indemnify Bones for any and all such liability, claims, suits, actions, losses,
    injury, or damages.

    I AGREE TO ABIDE BY THE RULES AND REGULATIONS AND ACCEPT ALL THE TERMS,
    CONDITIONS, AND STATEMENTS OF THIS AGREEMENT AND CONFIRM THE
    TRUTHFULNESS OF THE CONTENTS OF THE APPLICATION FORM COMPLETED BY ME.
    I CERTIFY THAT I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY
    UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS CONTRACT CONTAINS A
    RELEASE OF LIABILITY BETWEEN MYSELF AND BONES.

    of arrival and the day of departure as well as the days in between. RATES SUBJECT TO
    CHANGE WITHOUT NOTICE!!!!!
    **9am to 4pm-Monday, Tuesday, Thursday, and Friday (Departures only)(Arrivals 9am to 12pm), Wednesday-CLOSED (We are still here caring for guests), 9am to 12pm-Saturday, 6pm to 7pm-Sunday. Monday through Friday (Guest potty breaks 2:30-3:30. There will be a wait) We are CLOSED on all major holidays, yet still here caring for your dogs. NO EXCEPTIONS! PLEASE CALL IN THE EVENT OF AN
    EMERGENCY!
    **I understand photos taken at Bones, are the property of Bones and are a
    courtesy and an amenity offering a view into BonesLife while I am away. I understand
    photos are not a priority while my dog is in the care of Bones LLC, but that care is of the
    utmost importance! PHOTOS MAY BE PURCHASED UPON REQUEST!
